Justine is the Tributes' physician, so she will meet them all, but I don't have the time to thread out every doctor's appointment, especially when she's just starting out and forcing everyone into a physical.

Here, please make a header comment containing your character's medical history and any attributes that Justine is likely to make note of. Beneath that header, please leave a comment any time your character has a serious enough injury/illness outside of the arena to warrant a visit from the doctor. We can thread these exchanges out, or we can handwave it, whatever works best.

Condition: Definitely fit. She's developed decent muscle throughout her body and manages it well. She's pretty healthy all around, not having to go without much before.

History of Disease: As far as we know, she's been a healthy kid. Who knows what would happen if she were put into health/disease-risky circumstances, though.

Have they recently experienced any life changes or unusual stress?: Just the weight of the world! But she won't say that and will awkwardly opt to "just... what's expected when you're here."

Sexually Active? If so, how many partners within the last year?: Nope! Gary's had sex a grand total of one time, sophomore year, and he didn't enjoy it all that much. If pressed, he'll say that he's been on a lot of dates, but won't go into any more detail.
Alcohol and frequency thereof: Not unless others are drinking with him (e.g. at parties)
Deaths: None yet!
Condition: Healthy and built like an athlete--lean, muscular but not especially toned.
Medication: No medication needed, not currently in therapy (and with no desire to start)
Immunizations: Gary's up-do-date on all standard Earth vaccinations given out during the late 80's through early 2000's. If that isn't enough, he might complain a little about getting more shots but ultimately won't fight them.
Medical history: Other than a few non-critical fractures and maybe a minor concussion earlier in life, nope--nothing that's required surgery, at least.
History of Disease: Gary's immune responses are relatively normal; minor tree pollen allergies, and he might get a cold or the flu every few years, but nothing drastic.
Emergency Contact: Gary will list his Aunt Debbie and his brother Clayton, but of course neither of them are in Panem so really, none.
Have they recently experienced any life changes or unusual stress?: Hah! What a funny joke! Gary will laugh at this and not actually answer the question.

Demeanor: Gary will be very friendly and cooperative for the whole thing; if he's ever uncomfortable it's only when he's asked a question he doesn't want to answer, or if Justine has cold hands, and in any of those cases he'll dismiss his discomfort with a joke or a change of subject. Justine might not be trying to flirt with him, but Gary will try to casually flirt with her. About midway through the physical he will get very fidgety and chatty and will probably have to be told multiple times to sit still.
Other: Not that I can think of! EDIT: I guess if Gary's abilities are ever restored for some reason, he'll have really odd blood samples, but I don't think this will be relevant for a while.

Deaths: He's never died, but he's been injured several times to varying degrees. Beaten once to an unconscious state, tranq'd once to a blissfully asleep state.
Condition: Regular weight and cardio training has put him in fantastic shape. He's a little bulkier than his siblings, but all muscle.
Medication: Nope.
Immunizations: Nope, and he'll vehemently refuse.
Medical history: Nope nope.
History of Disease: Just your average stuff-- common cold, flu, etc.
Emergency Contact: Donatello and Leonardo... probably Donnie first, though. This Raphael has a particularly strained relationship with Leo, though that can change over time here, of course. He's no fan of letting Leo see him as "weak", and will often go out of his way to prove he's somehow superior. He feels much less rivalry toward Donatello.
Have they recently experienced any life changes or unusual stress?: He'd likely just write "seriously?" here, and if asked face-to-face, respond by inquiring how one gets a medical degree by being oblivious to the obvious.
Special Needs: None at the moment!
Disabling Conditions: I don't know if I'd call it disabling, really, but he's got a terrible temper that's interfered with his judgment multiple times in the past.
Notable Scars: Eh, flecks here and there from fights and getting tossed about, but nothing in the way of major skin scarring. His shell has a rather impressive assortment of unnatural grooves and scuffing, with one particularly deep wound near the upper left.
Other Notable Attributes: Nothing aside from being what he is-- a big turtle with human attributes.
Demeanor: She'd really have to push him to receive a violent response, but he would definitely be defensive and difficult to deal with. Basically, if he's not bleeding out, he won't want to be touched or questioned. Raph likes humans, and will forever strive to protect them, but he's also very much a believer in personal space. If he can walk out, he will.
Other: I don't think so! I mean, he's a big turtle. It's weird.

Approximate Age: 50
Date of Birth: September 26, 1984.
Sex: Male
Sexually Active? If so, how many partners within the last year?: IC answer: "None of your goddamn business." It's probably easy to find out that he has slept with fellow tribute Jane Shepard, but otherwise getting him to talk about his sexual activity is like pulling teeth. (Shepard is the only one he's slept with in the past year or more, though.)
Alcohol and frequency thereof: Occasionally, but never to the point of drunkenness. Maybe 2-3 beers a week.
Deaths: Yes. In his first arena he was killed by the Creature (just beating each other to death as I recall), in his second a similar thing happened with one of the Buckys, and his third was, uh... I don't think I ever established exactly how he died, woops. Probably just "accidental".
Condition: Pretty hale and healthy for his age and previous living circumstances, though when he first showed up (and thus when he gets reset after every arena) he suffered from slight malnutrition. He's got a lot of scars on his body, and probably some internal scarring too from that time he got impaled on some rebar.
Medication: Nope. He could probably use some therapy but he would never agree to it. XD
Immunizations: Yeah, he was up-to-date on all that stuff through 2013 America.

History of Disease: Nada.
Emergency Contact: Tess, his business partner from home. They've known each other for something like a decade and he trusts her more than any other adult here.

Notable Scars: He has a lot of scars all over, but the biggest one is on his lower right abdomen, front and back, from when he fell and landed on a piece of rebar.

Demeanor: Pretty sullen, if not downright hostile. He hasn't had a formal doctor checkup/physical in over twenty years, and he seriously mistrusts any doctors and authority figures, especially Capitolites. He wouldn't fight her but this is clearly a guy who doesn't like and isn't used to being poked and prodded. His answers to her questions will be as terse and monosyllabic as he can make them - while he wouldn't outright lie, he also has no interest in disclosing his personal history and would have only given out the most basic of info to her.

Alcohol and frequency thereof: He's probably the wine-with-dinner type, perhaps a nightcap from time to time. Because, you know, medical studies say red wine is healthy.
Deaths: First death: stabbed in the eye with a knitting needle.
Second death: attacked by rats, Initiate Fraysong put him out of his misery
Third death: shot by a sniper then stabbed to death with a plastic candy cane.
Condition: Middle age spare tire of pasty nerddom, but as healthy as he can get himself by reading medical journals and following advice.

Demeanor:He's a doctor and we all know doctors are the absolute WORST patients. The worst. So he'll probably find the whole thing, being treated like 'meat' to be somewhat offputting, and at least internally be critiquing her bedside manner.

Sexually Active? If so, how many partners within the last year?: Since he's a preteen he'll be getting the "wet dream" question instead. The answer will be "nine" without hesitation but then he'll gleefully inform her he'll probably be having one tonight, too.
Alcohol and frequency thereof: HE SHOULDN'T BE but he will imbibe anything he can get his dirty little hands on.
Deaths: Not here yet, but like 90-100 times in canon. However, he's keeping that aspect of his life to himself... for now.
Condition: Skinny and pale, on the underweight side.

Immunizations: I'm gonna go with no because his parents suck.

Sexually Active? If so, how many partners within the last year?: Yes, two. A boyfriend and a girlfriend. He's not been active with his boyfriend since he arrived here in January, but his girlfriend visited during a plot, and they certainly were active then.
Alcohol and frequency thereof: Yes. Joly's not an alcoholic but he drinks frequently, especially wine and brandy, common for someone of the 19th century
Deaths: Joly died before arriving in Panem, due to a bayonet stab to the gut. It was horribly painful and gory. In Panem, he's died in the arenas, first beaten to death by Zoidberg, then poisoned by poison bug, and breathing in poison debris during the collapse in the recent mall arena.
Condition:Joly is asthmatic, both environmental and stress induced, has a weak heart and constitution, and is a hypochondriac. This happens to medical students who had no understanding of germ therapy until Panem and believed diseases were caused by miasmas.
Medication: None.
Immunizations: No. What are those...why are you putting disease into his body? He's probably actually going to insist on avoiding these things if he can.
Medical history: No medical procedures
History of Disease: Joly was born early, with exceptionally frail lungs and a weak heart. He was not expected to survive to age 7, and was frequently ill with pneumonia, has severe asthma and a generally weak constitution, though he's rather faster than his lungs should allow him to be, having learned to work around them. He also contracted the epidemic disease rushing through Panem several months ago.
Emergency Contact: His friend Courfeyrac, at this point, or Venus Dee Milo, a friend he's made here.
Have they recently experienced any life changes or unusual stress?: Joly would look at her at this question and burst out laughing.
Special Needs: Jply could probably use an inhaler, though he hasn't done so yet in game
Disabling Conditions: The hypochondria, really. Joly's nervous condition tends to result in a lot of worrying, until he is able to go into doctor mode himself.
Notable Scars: A long scar from a bayonet on his lower abdomen

Demeanor: Joly would want to avoid the whole process, as he knows what he is doing, thank you very much, but, once he was eventually hauled in, he'd flirt back, and be rather interested in the medical knowledge she has gained. He's been studying from some books thanks to our very kind liaison, but seeing things firsthand would rather fascinate him. And he'll likely be one to drop in often if she'll permit it.

Date of Birth: Never specified. I'll edit this when I've thought it over and come up with something that makes sense.

Sexually Active?: The apocalypse puts a damper on things. But he's been lucky enough to have had sex at least once in the past year.
Alcohol and frequency thereof: Now that alcohol is more readily accessible, he'll have a beer or so every couple of days. His carefree days are behind him and he's mindful not to overdo it.
Deaths: Hasn't been in the arenas yet!
Condition: Able-bodied and reasonably fit because he's used to doing quite a bit of running away from walkers. He's on the thinner side since food is scarce where he's from.
Medication: None.
Immunizations: He was up to date before shit hit the fan roughly two years ago in his world.
Medical history: Nothing super major but I bet he probably broke his arm doing something dumb in his teen years.
History of Disease: The worst thing he's ever suffered is probably walking pneumonia -- once as a kid and likely something that would have crept up on him after suffering injured ribs in canon.
Emergency Contact: Nick -- Luke's best friend of twenty years. Luke would hate to put him in a stressful position like this, but yeah.

Disabling Conditions: Nope.
Notable Scars: A bit of scar tissue on his lower right side reminding him of Carver busting his ribs (though that healed up upon entry in Panem). Scar where bullet entered his left leg (this also healed). Other than that, a small collection of scars all over mostly from childhood and adolescence.
Other Notable Attributes: Is infected with a virus that causes him to reanimate after death regardless of how he is offed. Unless his brain's damaged.
Demeanor: Luke hasn't had an exam like this in at least two years and he's not too thrilled about it especially knowing it has everything to do with ensuring he's in tip-top shape for this human bloodsport. Plus, egh, hospital gowns. But he'll cooperate and put up with it mostly because he wants to be healthy for his eventual escape-attempt. Plus he's probably a little curious about how he's doing after so long and living in squalor. While it depends some on the mood he's going into it with, he might be terse with his answers (but not unkind), though he could warm up a bit at her friendliness and joke lightly. He's not used to a female doc and it'll be a bit weird for him to hear the flirting. Now he likes being flirted with as much as the next guy but he'd much rather she be professional while having a look at him. Outside the examination room is another story, though.

Approximate Age: Nill is about 19, but closer to 21 mentally due to being bumped down in age. If asked she'll probably say 20.
Date of Birth: She has no idea. She celebrates it August 28th.
Sex: Female.
Sexually Active? If so, how many partners within the last year?: oocly, no, but she's canonly dealt with sexual abuse. None in the past year I mean, but idk if she'd do anything that might result in discovery of that. icly it is also a very quick and immediate "no."
Alcohol and frequency thereof: She does drink but she's been trying very hard not to in the Capitol. Last time she had anything to drink would be cited as during Clara's crowning.
Deaths: For this one Nill would just tell her that "nothing has happened to her since she arrived." Prior to thegames however she died twice, nearly died a few times, and had to have emergency surgery at least twice, and had blood transfusions for one of those.
Condition: If not for what little muscle she's been able to put on then she would be underweight. As of right now she's pretty okay, but should probably sleep more than she does.
Medication: No medication, and no therapy.
Immunizations: Honestly, probably not. She'd be a little confused by the question, but ultimately concede that she doesn't know.
Medical history: Two surgeries, which would be cited as fixing some very badly broken ribs and resetting a few bones in one of her wings, and there's a spot on one of them where the feathers don't grow quite right that she might indicate if need be. The other she'd just mention as "an accident," and she was in the hospital after.
History of Disease: She's prone to getting minor colds, and will mention two very bad cases of the flu, which were actually game plagues.
Emergency Contact: Harleen Quinzel and Mindy Macready, because they're her mentors. If questioned on it she would say that she doesn't have anyone else she'd feel comfortable having as her emergency contact.
Have they recently experienced any life changes or unusual stress?: She'd mention being uncomfortable in the Capitol, but not go into any more detail than that.
Special Needs: None!
Disabling Conditions: She's mute! Otherwise she physically has none. Mentally speaking there's some pretty obvious PTSD. She can also become extremely depressed, but it's usually because horrible shit has happened.
Notable Scars: A very, very faded one at the base of her throat that you can barely see, the sort that can sometimes come from an operation. There are also three very long and ugly looking ones across one shoulder and down to her hip and slightly on her side. They're the kind of injury people don't usually walk away from, and despite then running over a wing, the wing is oddly untouched. Her hands also have scars between the fingers and on the sides, and there would be several on her palms as well, but they were mostly worn away, so there's only a few left. Also a bullet wound scar on her chest, left side, below her collar but not low enough to do any real damage to her boob. Pretty sure that's all of them!
Other Notable Attributes: The wings are a pretty obvious one. Also, if Justine checks her out closely enough, she'll probably notice that Nill doesn't have vocal cords. I'm not sure it would get to a point where Justine would be doing x rays, but if she did she might notice that there's another chip in Nill's head, though no longer functional. But since the capitol put theirs in her head, it might not be there any more, or she might have documentation of it already. It's probably in the part of the brain where memories are stored.
Demeanor: She definitely makes an attempt at being polite and okay with the situation at the start, but it doesn't last long once the actual physical part starts. This has tapered off dramatically as his physical condition and disposition have deteriorated; the people who have been interested in him that way between five odd years ago and now are concerning and have obvious problems and issues of their own. Whether in an interview or in the privacy of a doctor's appointment, Linden's blunt, unflattering attitude on the subject doesn't change much. "It's just a lot of tedious rutting, isn't it?" Followed by mean-spirited laughter, followed by a jaded, bitter, and uncomfortably quiet smirk. While he wouldn't give a straight answer, it's very unlikely that he's had a partner within the last several years, Capitol-arranged or otherwise.
Alcohol and frequency thereof: On a scale of Never to Haymitch, Linden is a heavy drinker. If it weren't for his other, worse vice, he'd almost definitely be considered an alcoholic.
Deaths: Two near-deaths in the arena a long time ago: the first via poisoning by a stalactite scratch, which he recovered from with rest and sponsor-sent medicine. The second time, his throat was cut by the last remaining victor; it was a wound that would have killed him, but he lasted just long enough to see the other guy die first.

Approximate Age: Apparent age is a fairly old and weathered 55-ish, about ten years older than his chronological age, but he doesn't really consider that sort of thing. And if he looks old, he feels old too, so he wouldn't mention it if asked about his age.
Date of Birth: If asked, he'd probably just shrug and say time in his world was fairly unreliable when he was born. As far as I know his culture didn't have a year numbering system, as one is never mentioned.

If so, how many partners within the last year?: He'd probably say one since he came to Panem, and get irritated if she pressed for more because it's difficult for him to remember specifics about the probably-a-year he spent before Panem. Honestly, it's been about four, three in the year-ish before Panem, one after.
Alcohol and frequency thereof: He'd say he's probably had one or two drinks since he's gotten to Panem, but no more than that.
Deaths: He was thrown off the ferris wheel his first time, and fell from the third floor onto the ice rink in his second. The mini arena was never played out but I like to think he fell from something there too, because I'm planning on having him die that way every time.
Condition: There might be a few signs that he's gone without enough food or nutrition for periods in the distant past, but - maybe partly through physical ability, but definitely through a learned technique - he's able to survive longer than most people without food or water (something he wouldn't mention), so the times he's had to go without may not be super obvious. He's got good muscle tone and reaction time (tbh he might try to tone the reaction time down a little where he can, so it seems more normal than it is) and is overall pretty healthy. Whenever he gets sick in canon it seems to settle into his lungs, but I know nothing of medical things so I don't know if that means there's something there she'd be able to detect or not. I figure it's because of some sickness he might have gotten as a kid, but again I know nothing of medical things so.
Medication: He is not.
Immunizations: Nothing beyond what Panem might have automatically given him when he got there, which I think means probably not any.
Medical history: The only surgery canon talks about is major injuries being repaired by a certain type of supernatural bug crawling all over him, because Stephen King, but I don't think there will be any physical evidence of that and he wouldn't think to mention it.
History of Disease: Not allergic to anything. Possibly that maybe-headcanon childhood illness I mentioned earlier, but not much after that. He'd mention that he got an infection when his fingers were bitten off, but he probably wouldn't mention that it very nearly killed him. He'd be vague otherwise, because though he did almost get sick one more time (something that settled into his lungs, of course), that would be one of the things he doesn't remember well. In Panem, he got tuberculosis at one point and once it was identified, he got Panem's treatments for it. Whatever those were.
Emergency Contact: At the time she arrived he probably would have said the Signless - they each have different definitions of their relationship, but on Roland's part it's a deep loving friendship thing. He would skate over that if she asked, probably just shrug and say they were friends.

Disabling Conditions: Where his right big toe and the index and middle fingers of his right hand used to be there's uneven scarring where they were bitten off (not even stubs left, just scars), but he's learned to use what he's got to do everything he needs (except to use weapons with that hand, or to use any weapons that require both hands with much precision).
Notable Scars: He has a lot of those, especially if she insisted on seeing him with his shirt off. They're mostly across his chest and back, with a few on his stomach, shoulders, and biceps. It'd be fairly clear he's been shot more than once, almost-stabbed, cut, and at least one point whipped. And of course, the messy brand Panem gave him over one cheek after the failed jailbreak.
