Doctor's Notes

Justine is the Tributes' physician, so she will meet them all, but I don't have the time to thread out every doctor's appointment, especially when she's just starting out and forcing everyone into a physical.

Here, please make a header comment containing your character's medical history and any attributes that Justine is likely to make note of. Beneath that header, please leave a comment any time your character has a serious enough injury/illness outside of the arena to warrant a visit from the doctor. We can thread these exchanges out, or we can handwave it, whatever works best.

Alcohol and frequency thereof: He drinks, but rarely. Usually he'll go out for a drink or two after an arena, and he's shared wine with Cecil, and the period between Cecil's avoxing and his own arrest was a little bad, but normally he doesn't.
Deaths: Two. Once by strangulation, once by impalement and a broken spine from falling down a flight of stairs.
Condition: More fit than anyone who spends all his time in a lab should be. He didn't earn those toned arms. How did he even get them?? He has cat allergies and the beginnings of a strange rash in the back of his throat, but otherwise is in perfect health.
Medication: No regular medicine, no. He probably should be in therapy, but isn't.
Immunizations: All of his were up to date in Night Vale, and he has submitted to any the Capitol wanted to give him.
Medical history: He's broken a bone or two, but otherwise, no.
History of Disease: Cat allergies. Other than that, Carlos is remarkably disease-resistant.
Emergency Contact: ...if anything happens to him, he'd like Steve Rogers or Thor Odinson notified. They're friends.

Special Needs: Nope!
Disabling Conditions: Nope, though being tortured and branded has left him a little less sane than he was. This insanity manifests as being really hard to shock or surprise. He's calm about pretty much anything. Super zen.
Notable Scars: Just the Capitol brand!
Other Notable Attributes: His hair is perfect. Even after weeks in the Arena.
Demeanor: Carlos would be initially chill, very cooperative, following instructions -- until she started flirting and he realized he was being sexually harassed. He would call out for behaving unprofessionally, and when it became clear she wasn't going to change her behavior, Carlos would close his mouth and submit to the rest of the examination -- still cooperative, but silent and stony-faced.
Other: Not that I can think of.

Alcohol and frequency thereof: Yes, a lot. She's not a full-blown alcoholic, but she drinks a lot

Condition: Extremely fit, toned, slightly underweight for her height

Immunizations: Current-ish? The Soviets actually had really good immunization programs, but Molotov wouldn't have added anything as an adult.
Medical history: Probably has had procedures to remove bullets, glass fragments, etc. No major surgeries.
History of Disease: Nothing special, just your ordinary chicken pox-as-a-kid style stuff
Emergency Contact: lol no one in Panem!
Have they recently experienced any life changes or unusual stress?: Other than The Games, Molotov will just say “new job” and not elaborate.

Deaths: One now! She was impaled through the lungs on a jagged metal post.

Full Name: Jane Shepard, Commander

Approximate Age: She celebrated her 32nd birthday birthday int he Capitol, but is slightly less than 31 biolgically. This gap will continue to grow for as long as she participates in arenas.

Date of Birth: April 11, 2154

Sex: Female

Sexually Active? If so, how many partners within the last year?: Yes, although she probably would say "more than three, let's leave it at that" to any questions of number.

Alcohol and frequency thereof: She usually has at least one drink for each day, occasionally more involved binges-- she would describe it as, "a beer every now and then".

Deaths: Once per arena, with an additional death from her canon. Asphyxiation, blunt force trauma, stabbing, starvation, and then some others, including a couple of maulings. She seems to be taking it well.

Condition: Shepard is a marine at or near her physical peak, except where stress and sleep deprivation have taken their tole. She's stocky, muscular, liberally speckled with the bullet-scars and burnmarks one might expect from a field-commander during wartime. She also has an interesting collection of glowing fissures or cracks in her skin that she refers to as scars, but which are almost certainly not scars. Any attempt, surgical or otherwise, to correct those blemishes will inevitably end in them reopening. Stress + Cybernetics = Terminatorface?

Medication: Shepard resists all attempts at medication until it is absolutely necessary that she take them, including pain medication, and would probably rather die than speak honestly to a therapist.

Immunizations: Shepard has received all the vaccinations and standard boosters one might expect from someone who plans to go to space and meet alien life forms and alien eco systems, as is standard for Alliance Marines.

Medical history: Shepard would admit to some cybernetic enhancement and "the usual gene mods, standard issue stuff," but would otherwise refuse to go into detail. They've had their hands on her corpse often enough that whatever they don't know, she's unwilling to add to.

History of Disease: Shepard has no allergies nor has she been sick in living memory.

Emergency Contact: "In event of an emergency, medical decisions default to next of kin, or in the absence thereof, the next in the chain of command. Right now, that's Garrus Vakarian." Her association with him is no secret; they've teamed up in every Arena they participated in and are well known to socialize together. Garrus also rescued Shepard's other known association, Thane Krios, during the jailbreak. It's safe to say that they are very close friends.

Have they recently experienced any life changes or unusual stress?: "Well, I was kidnapped by some insane zealots running a pokey, postage-stamp sized nation-state on a do-nothing backwater planet in the middle of nowhere, then run through the death-games a few times, so I'm gonna go with, no. This is completely normal. Nothing about this is a complete waste of time at all."

Special Needs: No special needs

Disabling Conditions: She's nursing a healthy case of PTSD that she will in no way acknowledge until the war is won. But that's unlikely to come out.

Notable Scars: A liberal speckling of bullet wounds, scarred over and well-healed, the aforementioned cybernetic "scars" as well as a prominent brand in the shape of the Capitol's trademark eagle burned into her forehead, slightly askew.

Other Notable Attributes: Shepard is at least 30% cybernetic, and as such is significantly heavier than an ordinary marine her size would be. Her eyes glow-- literally, they glow like the terminator. She would respond int he negative if any offer was made to open her up, and with slowly escalating violence if the issue were pressed.

Demeanor: Shepard's been through this before; regular physicals are a reality of military life, particularly when you get shot as often as she does. She would ignore flirting with blank looks and long, awkward stares, brusque and businesslike to the point of rudeness, making no bones about the fact that she is only here because it is required. You're not her friend, you're being paid to do a job, so lets just get this over with.

Other: Shepard has a cranial implant, which is externally visible at the base of her skull, hidden under the hair just behind one ear, near the nape of her neck. It is not obvious, but it is present and anyone running their hands through her hair would be likely to encounter it. This is her amp port, and appears no more remarkable than an inch-by-centimeter square of brushed-steel texture metal embedded in her scalp. It is possible to quickly and easily remove the biotic amp contained within, but that would require either specialized proprietary military software and tools, or Shepard's cooperation, neither of which is likely to be forthcoming. Shepard would call it "Standard Military implant. It can't be removed" if asked, and that is the absolute truth, if not all of the truth.

Full Name: Sollux Captor
Approximate Age: 13 when counting by human years. He'd list it as 6 solar sweeps.
Date of Birth: ??? None listed in canon and he wouldn't know how to convert it to a human calendar anyway.
Sex: Male
Sexually Active? If so, how many partners within the last year?: Nope. He'd thoroughly protest any invasive questions, too.
Alcohol and frequency thereof: Not particularly. He got sent gin as a sponsor gift in the arena, but beyond a brief sip (at which he decided ew, gross) he wouldn't have been interested.
Deaths: Not while here. He got punched up a bit by another tribute before the arena, but not badly.
Condition: He's a scrawny nerd, the sort of guy who doesn't take as much care of himself as he necessarily should and probably thrives on junk good when he's more on top of his game. He's had little reason to be physically active in the normal sense; until arriving in Panem he had psionics to rely on all his life.
Medication: None.
Immunizations: No idea for troll ones, but never against human diseases in any case.
Medical history: None.
History of Disease: No diseases or illnesses noted in canon, nor anything about his resistance to such. As for allergies, the closest would be that a substance called 'mind honey' (produced by the purple bees on his planet) causes a violent reaction when he has his powers, namely giant eye lasers without his control. Normal honey would probably not affect him the same way, but he'd avoid it nonetheless.
Emergency Contact: Feferi Peixes. She's his matesprit, which is a troll form of romance that maps closely to the basic human form. They've been dating for an indefinite length of time, but less than a month before he arrived here.
Have they recently experienced any life changes or unusual stress?: Sollux would blow up on her for this one. Being brought to the Capitol, being shoved in the games, and that's on top of stresses from his life before here, which he wouldn't want to go into.
Special Needs: Nothing in particular.
Disabling Conditions: Sollux is described as "bipolar", though he acts more like the pop culture concept with frequent moodswings than any real, accurate depiction. He used to suffer frequent migraines, but this has become a non-issue with the absence of his powers.
Notable Scars: None.
Other Notable Attributes: Considering he's not human and unusual even among trolls, pretty much everything. Grey skin, horns (four of them in two matching pairs), oversized fangs (which cause him to lisp), yellow blood, a body temperature slightly but tangibly cooler than humans, pointy ears, lack of body hair (he's not a mammal despite the humanoid body plan), no navel at all (ditto), and unusual eyes (one is red, one is blue, neither with visibly discernible division between irises, pupils, and sclerae).
Demeanor: He'd be visibly uncomfortable the whole time and vary from quiet and cagey to explosively angry. The more handsy she is the more irritated he'd get. At worst he might try to bite her, but odds are it would take a lot to get him there given he knows it would only bring him trouble.
Other: He's actually died once before coming here via violently hemorrhaging blood from every orifice thanks to the psychic scream of an enormous tentacle monster, but revived without trouble in a spare body, so there would be no sign of it at all even if he brings it up. (His canon is weird, okay.)

Full Name: Sigma Klim
Approximate Age: 67 physically, he would report 69. ~~He's a bit older thanks to time traveling shenanigans and also having been here for two years while being reset to his normal state every death but that doesn't really matter~~
Date of Birth: July 18 2006
Sex: Male
Sexually Active? If so, how many partners within the last year?: One partner in the last year.
Alcohol and frequency thereof: 1 or 2 drinks a week, tops.
Deaths: Been in the Arena six times: Fallen off a cliff into ice water, been stabbed to death, hacked apart with a machete, died of influenza/strangulation, beaten to death, jugular ripped out, drowned/electrocuted by cybernetics.
Condition: Physically, not bad for someone pushing 70. Very muscular and fit for his age from working out every day. Has a very poor immune system from being locked in a building for about fifty years, though.
Medication: None.
Immunizations: None recently.
Medical history: In the Capitol, was treated for a stab wound in the summer. In canon, 45 years ago, his arms were lost in an attempted murder of sorts, and replaced with titanium cybernetics with false skin and muscle, and his right eye was gouged out and replaced with a cybernetic camera.
History of Disease: Extremely vulnerable to disease. Caught and died of influenza a few Arenas ago. In canon, immediately before he was brought here he had recovered from a viral infection that caused insanity, suicidal thoughts and changes in perception.
Emergency Contact: Number one contact would be Homura Akemi. Very close to her, time traveling friend, pseudo-daughter. Second contact would be Sigma's "adopted" son, the Initiate, who is only second because he has been dabbling in Rebellion related things recently that Sigma doesn't want to be associated with at the moment. ...Also because he's a troll.
Have they recently experienced any life changes or unusual stress?: Publicly came out as an ex-Gamemaker to harsh criticism. Grew extremely close to the Initiate and then publicly condemned him for being Rebellious, on camera. Had a bad falling out with his "adopted" daughter, Eponine. Lost contact with his life partner a few months ago and is still worried for her. His biological son died in the Arena last year and he's still not over it.
Special Needs: The blood in his arms is made of a white artificial oily substance called "ABT fluid", Sigma can give the "ingredients" if they need to be replaced. His cybernetic arms diffuse medicines at a much slower rate than a normal human arm, to the point where it takes about 20-30 minutes in canon for an anesthetic injected into his arm to take full effect.
Disabling Conditions: Unless cybernetics count, none.
Notable Scars: A thick scar along both shoulders where his cybernetic arms were grafted onto his skin. Is missing an eyebrow, there is scar tissue there from skin being ripped off of his eyelid and eyesocket.
Other Notable Attributes: Cybernetic eye with camera (uncovered by artificial skin), titanium cybernetic arms (covered by artificial skin), tanned artificial arms but pale real skin. HERE is what the skeleton looks like, but his arms appear outwardly like so: HERE. If she wanted to open his arms and see how they worked, she would have to cut through artificial skin, which Sigma can feel. Sigma would have reacted VERY badly because he is terrified and disgusted with having his arms or eye touched, especially his left arm and especially with knives. He would eventually let her with some convincing or forcing, but would not be in a good place afterwards. If you want her to do this, we should thread it, because it would be character changing for at least a month. :)
Demeanor: Sigma would be extremely respectful, kind, open and honest so long as she didn't touch his arms or eye. He would immediately become quiet and prickly once she did, but never to the point of rage or violence. He would also blush and act a little evasive once she asked about his sexual history, because he's older than his lover and shy about it.
Other: I'm planning on Sigma becoming depressed as his pro-Capitol facade goes on. Also I'm sorry my character's medical history reads like War and Peace. Let me know if you need anything clarified.

Justine may force him into letting her examine his eye or arm, or she may back off and see if she can't manipulate him into consenting to let her examine it at a later date. It depends on the impression she gets from him, and whether or not she's likely to be able to turn him to her own cause if she cuts him slack.

Full Name: Prince Zuko
Approximate Age: 16
Date of Birth: 83 AG
Sex: Male
Sexually Active? If so, how many partners within the last year?: OOCly, I don't want to godmode it if we ever get a Mai but I will hesitantly say yes, within the last year. ICly, you could not get that info out of him with thumbscrews.
Alcohol and frequency thereof: Generally no, maybe on special occasions. He's more a tea kinda guy at this point.
Deaths: Not yet but he will have one by the end of the arena
Condition: Zuko is super duper crazy physically fit, fairly strong, and incredibly agile. Peak human condition kinda dealy. The only problems he's dealing with are slight malnutrition. He had some time to eat well when he was back in the Fire Nation, but then he went on the road again and while he's been better with the Gaang than he had on his own, he went a while there hardly eating or eating refugee food. His body is likely still catching up on nutrients and will keep resetting to a slightly malnourished state every time he gets revived. (After which, he'll eat like crazy since he doesn't have to ration it with the Gaang).
Medication: No.
Immunizations: No.
Medical history: No surgeries.
History of Disease: No, Zuko has been sick only rarely. He got sick once in canon from running himself ragged, otherwise I figure he's very hardy.
Emergency Contact: Aang.
Have they recently experienced any life changes or unusual stress?: Zuko will stare and not even dignify this with an answer because where does he even start.
Special Needs: None in particular.
Disabling Conditions: Zuko is sliiightly mentally ill in a "I seriously need some time or therapy to deal with my daddy issues" way. He had a shitty childhood and a rather traumatic last few years and he does have anger management problems and trouble with mood regulation. He's better than he was, but he's still not 100% mentally healthy. Also I play him as mostly blind in his left eye other than seeing vague shapes. He just deals with it.
Notable Scars: Uh, weeellll... Other than the facial scar, I figure he's got little ones all over his body from a hard life. After all we see him get fairly beat up over the series, as well as minor, tiny burn scars on his hands and feet - something fairly common for firebenders because accidents happen when they're young and starting out and he was particularly bad at it. But it's sort of like how a blacksmith might have a few burns here and there.
Other Notable Attributes: None in particular.
Demeanor: Cranky, putoffish, somewhat rude though he'll be smart enough to not attack or take it too far. Occasionally outraged at what she's asking (asking the Crown Prince about his sex life???). He'll stick to a lot of monosyllables

Full Name: Hiccup Horrendous Haddock III
Approximate Age: 20
Date of Birth: Maybe sometime in a variation of Earth's 800s? 900s? Maybe the early 1000s? The Viking age.
Sex: Male
Sexually Active? If so, how many partners within the last year?: Thoroughly so, one partner for the last few years. He may accidentally brag slightly about how awesome his girlfriend is at that whoops.
Alcohol and frequency thereof: At home, he drank a little bit of weak alcohol every day because the water wasn't always the best, but he doesn't drink much in Panem.
Deaths: Chopped by a rusty knife in the heart, still hasn't died this arena yet.
Condition: Kinda skinny but in fairly athletic shape. He's not in as good a shape as the hardcore soldiers and warriors but probably in better shape than the average modern 16-year-old due to the Vikings living very active lives and constant adventuring. Rarely gets sick, apparently has an immune system that let him lose a leg and survive without dying from infection in pre-modern times. The worst problems he has are amputee related - phantom pain, sometimes muscle pain in his leg.
Medication: None
Immunizations: None.
Medical history: When he lost his leg they probably had to cut away the burned flesh and stuff so that was pretty nasty. I play it that he was zonked on herbal rememdies during that and the whole time he healed.
History of Disease: Very resistant to disease, to the point of surviving a dismemberment without modern medical treatment.
Emergency Contact: Astrid, his girlfriend who he's kind of nuts in love with, or if she's not available, his friend from home, Ruffnut.
Have they recently experienced any life changes or unusual stress?: Other than side eyeing her because The Games, he would mention he lost his dad and became chief but not elaborate much on it. He was also tortured and branded in the Penny plot but he wouldn't talk about that at all.
Special Needs: Amputee. Needs a leg prosthetic.
Disabling Conditions: Same. Amputee, missing his left leg.
Notable Scars: Aside from the leg, he does have some minor burn and cut scars on his hands and arms from blacksmithing, as well as a little scar on his chin. He also has his brand on his forehead from being tortured, though since he was tortured mostly with hallucinogenics and electricity, he has no other notable scars from that.
Other Notable Attributes: The prosthetic is pretty simple if clever but not that mechanically complex. Also, it's a different medium but I figure Hiccup is almost proportioned like he's a baby undersized giant. Like he's 6'1" but looks like a person that grew up undersized that should have been huuuge rather than a large person. Compared to a lot of the other people there, I think his hands and feet, for instance, would be enormous. It would give an idea of how big his people generally are that he's really tall but looks proportioned like he was meant to be much bigger and missed a growth spurt or something.
Demeanor: Generally amicable and sarcastic, slightly nervous, probably would not stop talking to the point of possibly being annoying. Would actually be proud of the prosthetic if she looked at it and explain how he designed it.

Date of Birth: November 12, 1994
Sex: Female
Sexually Active? If so, how many partners within the last year?: Not within the past few months. Only one partner in the past year. ICly, Clara will be really hesitant to talk about it because sex with her husband so isn't happening ever again, probably in the near future. Two within the past year. Clara will still be hesitant to talk about it, but not as much as she would have been prior to dating Rose.
Alcohol and frequency thereof: Drinks on occasion, but rarely overindulges.
Deaths: One so far. Was stabbed in the stomach before being mercy killed.
Condition: Fairly fit and healthy.
Medication: None.
Immunizations: Standard immunizations.
Medical history: Overall fairly healthy. Most major surgery she's had was a C-section.
History of Disease: Had chicken pox as a kid. Pretty much the standard stuff.
Emergency Contact: Her husband Alex. If he's unavailable, consult Dennett Norton (or Dave Strider).
Have they recently experienced any life changes or unusual stress?: OOCly, there's that whole being dragged away from her son when she thought her husband had just died to fight in a death arena. On top of that, four to five months prior to being brought to Panem, her husband was nearly killed by a car bomb and turned into a cyborg to save his life. Shortly after his return, Dr. Norton was forced to neurosuppress him (basically flatten out his dopamine levels to make him more compliant and act more robotic) and Clara was basically cut out of the loop and wasn't allowed to see or speak to her husband in that time. And then, just to make things even better, the Capitol has recently neurosuppressed Alex again so Clara isn't a happy camper at the moment. Update: And then she won an arena she didn't even want to win with debilitating injuries, so definitely not a happy camper at the moment. ICly, she'll grumble and evade at anything along that line of questioning.
Special Needs: None
Disabling Conditions: ~~None~~ She's currently missing her right eye due to losing it in the arena she won.
Notable Scars: A scar from her C-section on her stomach.
Other Notable Attributes: ~~None at the moment.~~ She has a cybernetic right hand after losing hers at the end of Arena 11. She'll be rather self conscious about it and cagey about letting Justine examine it.
Demeanor: Initially she'll be wary, but polite because of the Capitol. Clara will try to shoot down any and all flirting by pointing out that, hey, she's already taken, and will get more evasive and frustrated the more she prods at personal stuff.
Other: Nope!

Full Name: Querl Dox/Brainiac 5. The Capitol actually does NOT have access to his given name, Querl Dox, because there was no home universe to view his life in and he went entirely by Brainy or Brainiac 5 in what they could have viewed since it was likely some team stuff.
Approximate Age: 22 going on 23, though a really fuzzy 22. Any chronal scans would indicate there has been some time fuckery where time around him was relative at various points. The Legion are all only able to guess their approximate ages due to weird effects from frequent time and interdimensional travel.

Medication: No, however, the white things on his cheeks are his personality inhibitors and they regulate his mental health. If they were to be examined they'd be so intricate no one in the Capitol would probably be able to figure out the tech, he'd explain it as having been made by a semi-sentient space anomaly that messed with he and some of his team-mates' heads (which is true), making them slightly crazy but for some reason giving them things to help them too, something they have never been able to understand why.

Medical history: Brainy has been severely injured in the past many times. The two most major surgeries/treatments were 1) when his mother nearly burned him alive with his own forcefield. Disabling Conditions: Brainiac 5 is legitimately severely mentally ill with a condition that doesn't really have a direct comparison for humans or even other Coluans since none of the others were as intelligent as him. He's so intelligent and his brain chugs so fast in so many directions that he needs his personality inhibitors to keep him stable by helping bring his brain more order. Without them, he shows symptoms similar to bipolar mania or schizophrenia: disorganized thoughts, delusions of grandeur, hallucinations, mood instability and so on. With the inhibitors he's fairly emotionally stable.
Notable Scars: The "seams" from his skin grafts, some very subtle scars on his stomach. Otherwise medical tech in his world was so good he doesn't have many scars, which would be notable in itself. Almost perfect skin everywhere else
Other Notable Attributes: While pretty much mammalian, he'd definitely have physiology different than most humans, including some redundant organs or organs that have no human equivalent. Brain scans would also be an indecipherable mess. His brain is so alien and so complexly structured most humans would have to study it for years to figure out how it works and how smart he is. He'd even be keeping his pulse from going too bonkers and his pupils wouldn't dilate when he lies since he can control his bodily functions to a degree. Beaten with fists, shocked, had her fingernails ripped off, starved, and her face branded right on her forehead with the Capitol logo.

Her eyes constantly darting around the room for potential escape or weapons.

Other: Her metahuman powers are a genetic trait. As such her entire body inside and out can turn into a living statue. This allows her to suffer extreme injury without bleeding until she changes back. It still hurts like hell but hurts less then it would. She's been seen to be able to survive in this form even after having her limbs blown to pieces but only for a short time. Hypothetically with training she could learn to repair her body with sand and stone but she's never had the opportunity nor the guidance to do so.

Sexually Active? If so, how many partners within the last year?: Yes, until the Capitol killed Orc. But over the past year, Orc, and possibly a few Capitol citizens who have actually wanted Eponine. She's not fazed particularly about doing that, though she hates it and tends to feel very, very ashamed afterwards. In general, all of the men her father made her go with - Eponine is a sometime -prostitute, in that she took letters to rich men, leaving her at their 'disposal'' rather than standing on a street corner. She also had sex with Montparnasse, both in alleyways and in his rooms. He was definitely rough, and had a fondness for his knife.
Period wise, probably early. 12ish maybe - but given she doesn't know her age, she's not going to be able to answer that!

Alcohol and frequency thereof: Eponine drinks 'like a sailor'. She has a sailor's mouth according to Hugo, and apparently drinks a lot. She favours cheap brandy, but wine, vodka - a new introduction - anything - she's drinking more and more to block out bad memories
Deaths: Eponine has died a lot of times in the Games. Draco Malfoy tortured her to death, slit her neck and left her to bleed out. In a subsequent arena, he placed the imperius curse on her and made her attempt to murder her then-boyfriend, Howard. She was killed before she could. She's been shot in the eye by Katniss after actually torturing Howard and being hamstringed by Suzannah. In the most recent arena, she was shot through the hand and the kneecaps by Molotov, before being shot at point blank range through the head. Before killing her, Molotov taunted her about being a thief and being responsible for Orc's death. After her death, Molotov wrote 'thief' on her head and strung her body up outside the cafeteria for all to see. She's also been branded by the Capitol. The brand is on her left cheek, and starts just beneath her eye, and has caught the corner of her lips. She's also been executed by the Capitol.

Don't know how I forgot this - eaten alive by zombies (R) three times. They were very traumatic experiences for her.

History of Disease: Games wise, she was infected with tuberculosis. Generally, Eps is pretty resilient. I mean, she ran round Paris barefoot, even in the snow, and doesn't seem to have lost limbs or digits to frostbite or suffered pneumonia or anything.
Emergency Contact: Eva Salazar. Eva is Eponine's adopted mother - Eponine received the brand on her cheek trying to rescue Eva from jail. She'd do anything for Eva. Failing that, Sigma Klim (though Eps would say no one). He was Eponine's adopted dad, until he revealed his past and she got the wrong end of the stick. But she still loves him a lot.

Most recently, she and Orc, her boyfriend, tried to break Eva out of jail. They were caught before they even got inside, and Eponine was forced to watch as Orc was executed in the most horrific way. Her cheek was then branded and she was given a second cuff on her wrist. After that, in the arena, she was murdered by Molotov (as above) after falling out with her remaining allies, Dave and Elsa, because she tried to pass the blame for Orc's death onto Dave.
Special Needs: None - but she has a slight difficulty reading, writing and with numbers.
Disabling Conditions: she suffers from hallucinations when she's hungry. She thinks she's mad.
Notable Scars: Massive ugly brand on her face, in the shape of the Capitol insignia. Beyond that, she probably has numerous scars and bruises from her life in Paris, and the beatings she received from her father and 'husband'.

Full Name: Kurloz Makara, by birth or "hatching". Answers to The Initiate Fraysong upon receiving of a title and discarding of "wrigglerhood" name at 7 sweeps/16 years old (earlier than is normal for his kind). If his real name is used he will become aggravated and point out that it is only for "Quadrants" (Troll Romantic Partners) and close friends to use. He'll find it very disrespectful and likened to unwarrented flirting. His true name is a secret for him to give.
Approximate Age: Biologically 17. Chronologically nearing 18
Date of Birth: Alternian sweeps don't translate directly to years, but approximately 2 years = 1 sweep. Beyond that he's from ancient times even for his planet, the date is unknown. Capitol could easily give him "Jan 19", the end of Capricorn, as a fake date.
Sex: Male
Sexually Active? If so, how many partners within the last year?: No, he hasn't been with anyone intimately, although he is dating Jane Shepard. ICly: He'll say "Of course not! Drones ain't made no rounds so why the fuck should he?" The drones being things that his homeworld's government used to force trolls into mating, so the genetic material could be passed, or else the failed troll would be killed. Initiate doesn't find the topic of sex to be a great one due to the societal pressure mixing badly with his penchant for avoidancy and distrust of others.
Alcohol and frequency thereof: Nope. Hates the stuff. Only soda for him.
Deaths: In panem he has died six times. The first by poison (that cause all mental barriers to breakdown before eventually killing them), the second his injuries consisted of a few lacerations and a horn snapped off at the base (highly dibiltating) until he was attacked by Raptoroos (Chupacabras) and then stabbed through the heart with a sword. He was injured outside his second arena during the alien plot where he suffered third degree burns up and down his arms. The second arena he lost an eye earlier on and sustained three cuts across his face, dying at the end of the arena by Katniss's arrows. The third arena and fourth death involved an altercation on the Museum rooftop, one that ended up being far too rebel sympathetic, and so when the Initiate fell through the glass and was impaled below, the Disciple was framed to have pushed him (despite not doing so). In his fourth arena he broke a leg taking down another tribute, lost his eyes to another when they were stabbed out with needles, and then died near the end when he was electrocuted and then dissected on death. His sixth death involved being trapped in a room and drowning as it filled with water, the first death he showed legitimate fear towards. Prior to the games he has been through a great deal of altercations that have left him heavily scarred (lacerations and bites. He did once break bones in the same leg he broke in arena and in one upperarm, but he has healed greatly in both respects in part of troll biology). At this current moment he has been avoxed, lacking a tongue and having cut vocal cords (though still retaining the troll part of chitinous windpipe), he had the dud-fins on his ears sliced off messily, and will be being shot in the leg, just in the shallow flesh bits.
Condition: He appears rather skinny and wirey, but it's all deceptive. He's actually quite fit and very strong. He is surprising resilient even for his kind.
Medication: No. (Though while in capitol, he had a short secret stint of taking morphling to control his rage issues as a highblood troll and would likely take to it again once he's found access. His "family"* has a proclivity for drug addiction and strong reaction to drugs.) *noting that trolls don't have traditional human families but his descendant is highly addicted to a heavy narcotic substance and his past/alternate/other self frequently smokes.

History of Disease: Incredibly resistant to all sorts of illness, very rarely gets sick, but doesn't see doctors ever.
Emergency Contact: The Initiate would likely not want anyone to know of his state at all. He believes such care is reserved for moirails (a form of troll romance which he no longer has in Games) and wouldn't immediately seek care from any other. He is much more likely to take care of himself alone. However, if he had absolutely no choice but to name people, he'd select Kurloz Makara, his other self and the only other one he'd truly be okay with making health decisions for him, Sigma Klim, who is liken-able to being Initiate's father figure and so could perhaps have say, and at last, Terezi Pyrope, whom he cares very, very deeply for but he would not want her worried or making health decisions for him.
Have they recently experienced any life changes or unusual stress?: He is more alienated outside of the arena than within, his culture being one of violence and cruelty, making much of the niceties and anti-brutality parts hard for him to understand and accept. Beyond the Games themselves, or his avoxing, Initiate's undergone a lot of changes in the past year in regards to his worldview and state of mind. The future that was going to be his is now one he regards as a sin to bear, as opposed to a glory. He has come to care about people, and for his efforts, becomes extremely distressed over the mere thought of losing them. He fear he will lose himself completely in such a case and after losing his moirail (again, troll romance, this one notably being to help with mental stability, calmness, and caring for each other) he immediately lost control and was only just stopped from a rampage, subsequently taking to morphling to keep himself at ease. He is of the belief that he can be cruel and strong, making him unhurt but alone, or he can care and be weak for it, letting him keep people but putting him a greater risk of falling apart should he lose anyone. He for now has chosen the latter with a great deal of fear with it. On top of this, he is a secret rebel now, something he fought against back on his homeworld and thought as scum, making a deep conflict in him for that. So, there have been plenty of life changes and added stress to his situation. Initiate, however, will offer a glare for her question.

Disabling Conditions: He could very, very likely be called into having PTSD even before his time in games though he wouldn't even have the faintest idea of what that is. He can have a rather high amount or paranoia and fear, although he doesn't outwardly react to it until it comes to boiling point. Initiate has a power over fear, one aspect of it being he can feel the fear of everyone and anyone so long as this ability is available to him. This alone has left a great mark on him, but with the further ability to manipulate fear, channelling it through himself, he has done quite a number on him mind. He will and has admitted casually to "cracks" and "holes" in his brain (or thinkpan, as he'll put it), or even outright saying it/he is "rotted". He can sometimes (not often! But still) experience breaks with reality and episodes of psychosis, usually this coming in with a severe breakdown of some sorts (which happens) and a high amount of Highblood Rage (Those in "higher" blood castes like him have colder blood, higher resilience to psychic affects, increased/abnormal strength, and a higher proclivity for violence, anger, and instability). He sometimes feels a loss in control but wouldn't say this. He pretends to be comfortable with what he will admit to but isn't actually as much so as he convinces himself he is. (Refer again to his using Morphling to try and calm his own rage and to pull away from his own feelin and the world in general.) He is extremely mentally unstable. Also, while all of his kind suffers nightmares (or daymares, as they are natually nocturnal), Initiate tends for ones worse even than is average and sleeps poorly for it.

Other Notable Attributes: His hands + lower arms have a mild mutation, one that he will note he just got last growth spurt (that his alternate self will not have due to his poor nutrition stunting his growth). The mutation is non-debilitating, merely a strangeness in the formation of muscle tissue and bones in the hand. His fingers are longer than they should be and while his upper arms remain thin and bony (and will remain so like that), his forearms are wider. This can be seen in the chart linked above.
He stands at 7 foot without horns, is incredibly lanky for it, almost stretched looking, and will one day shoot past 12 feet. He's got a massive overbite of very large fangs, and his ears are long, seemingly pointed but actually more goat-like when relaxed. There is underdeveloped fin on both of them that he usually keeps hidden under his hair-- they're duds, good for nothing. He features a vestigial grubleg scar on each side, which appears as a patch of raised blackened skin, a little rougher and home to a lot of nerve endings (very painful to get cut, same with the fins). He's got a pair of waving goat horns that will get more waves to them over his growth.
Demeanor:Prior to his avoxing, he'd have reacted aggresively, snarling and snapping and tending for threats (which he would not intend to be empty) whenever touched. Highly reactive and quite openly hostile. He does not like doctors and is of the firm belief that a real doctor is one whom kills their patients, ergo, if he is convinced she is reall he will believe she will try to kill and torture him. If touched in a manner that sets him off i.e. somewhere he'd deemed inappropriate (beyond the obvious, touching his horns would be outright refused) or could put him in a dangerous position (behind the back, tugging his hair, touching vestigial grub legs, or going near his ear-fins), he would have reacted with violence. Following his avoxing, he will flinch heavily and violently at any contact at all, to the point it will be troublesome to touch him. He will try to keep any distance he feasible can manage. But for the flip side, he will be less prone to violence. He may be quiet but he exude a distinct air of discomfort. Prior to his avoxing, with having grown up with growing fear power, he developed and trained heavily to withstand the fear's affects. With avoxing, so the conditioning could take, this all was taken down to make him react to fear again and so be obedient. Thanks to this, he no longer has defence against the effects of his own power upon his mind and if this were switched on suddenly, it would be overwhelming and could even be damaging, with being unable to dissociate the fear that lay in others from himself. Avoxing or not, he will avoid a doctor's visit at all costs.
-To Note: It may be noted that his life on Alternia built a very hard persona upon him that his Avoxing directly clashes with. While he was initially societally conditioned to be violent on first instinct, the avoxing means for zero violence, harming no one. Alternia built him to be stronger and harder, putting in a strong, the avoxing means subservience and meekness with no presence at all. As both learned aspects oppose and clash and destroy each other, the person who he used to be underneath will start to show more. How he responds and reacts to people may be more genuine, if hesitant. How this presents is yet to be seen, but he will once more be a different person than he was.
Other: The fear power which is called "chucklevoodoo" often works along with vocalisation, adding a distortion to the voice that can be noticed when in use. Because Initiate overused his without proper training, his voice is permanently stuck in alternating volume. He can talk normal or whisper on the quiet part of the distortion, and he can shout or sounds like pushing words out through a harsh growl. He cannot actually help the fluctuations-- trying to do so, for him, would feel like gargling nails-- and becomes highly aggravated when people bring it up and/or tell him to stop. (And he might possibly be able to "safely" dislocate jaw? His descendant is shown doing this once in canon in a moment of intense rage. Probably not relevant.)
ALSO, he is the second iteration of Kurloz Makara, the previous one also being in the Games. Initiate is reincarnated from the other Kurloz, the whole planet having been essentially reset. While "Beforus" was peaceful, "Alternia" is violent, Initiate hailing from the deep past of the latter, and the other Kurloz coming from modern era of the former. They've lived very differently lives and grown up to be very different people both in personality and a little bit physically. Initiate does not have memory of his previous life. By all means they will appear as twin brothers over anything. Perhaps Justine will find this interesting.
And one more thing: His lifespan can extend to 1500 sweeps, which is 3250 years when properly calculated (beyond 1=2).

Medical history: Wesker was infected with an experimental virus prior to be brought to the Capitol. The T-Virus is incredibly powerful and a high mutagen. Not to mention 99.8% fatal. Only him and one other person have survived infection and both of them were mutated to varying degrees.

Disabling Conditions: His senses are enhanced and in particular his eyes are very sensitive to light. Bright lights can be painful if not disabling.
Notable Scars: None. The T-Virus is a cure-all, making Wesker good as new whenever he's injured. (Though obviously this pretty negated by the Capitol.)
Other Notable Attributes: First and foremost, the eyes. Instead of human eyes, Wesker has eyes with elliptical pupils, like a snake's, and the irises are red and gold. They also glow with surges of adrenaline. Second, if she has watched any footage from his time in the arena, he has a mutation in his throat. Approximate Age: 16 in human years. In troll years he's 7.5 solar sweeps, though since he's unaware that he isn't aging as long as he's in the arenas, he thinks he's coming up on his 8th wriggling day.

Alcohol and frequency thereof: Nah.
Deaths: First death was the last of three beheadings of the mutant trolls during the museum arena, courtesy of Justin. Second was when because he was eating the bugs in Placid Hollow, he was resistant to their poisons, but nonetheless the toxins ended up building up in his system over a few weeks and made him quite aggressive before he lost motor control and then died of a seizure.
Condition: He's healthy but not very fit. It should be noted that he's visiting the gym and training himself up as best he can these days, but since Tributes are restored to the condition they were in when they first arrived in Panem after dying in an Arena, he has to start over every time.
Medication: Kankri isn't going to the Capitol doctors for anything; however, he's developed PTSD from the Arenas, and Venus has started sharing her sleeping pills with him. Justine can feel free to ferret that out with footage in the Tower or some such.
Immunizations: He was vaccinated against troll diseases, at least...

Demeanor: Kankri is very uncomfortable with people he doesn't know touching him, particularly as much as Justine will be doing; he's also going to be very uncomfortable with the lack of clothing. But he'll also be doing his best to be polite and pleasant and try not to show his discomfort too overtly. (He's probably going to fail at that last bit.)
Other: Kankri is a mutant by troll standards and a bit sensitive about it. Justine can probably pick up on that at some point during the physical. Oh, also, I am really really open to having her fuck with him in the future and would be happy to do a log for the physical if you'd like. Condition: He's a pretty scruffy, ragged-looking raccoon-like creature (he wouldn't use that word, though), but as far as health goes, he's doing alright. Eats decently, plenty of exercise, more muscle mass on him than one might expect, given his stature.

Medical history: This may take some prying, but yes. According to Rocket, he was surgically made. At the very least he was experimented on for a long time and still retains most of the scars and implants that were given to him.

That's kind of a life change and a stressor, though not anything he can't handle. Prior to that there was his sudden (and unusual) turn for the more altruistic behavior, but that's whatever.

Disabling Conditions: It's possible that he may have a bit of PTSD regarding the experimentation on him, but it's not an event that often comes up in conversation and therefore doesn't often affect him. Other than that, nothing.
Notable Scars: Beneath the fur there are plenty of scars all over Rocket's body. They're all primarily surgical in nature, though there are also some blaster burns over his sides and legs. Bits of metal can be seen emerging from his back and chest, remnants of surgical implants that were likely torn out rather than carefully removed. For the most part, the scars are very old.
Other Notable Attributes: He's a 4 foot tall not-raccoon.
Demeanor: Rocket would be gruff, irritable, sarcastic, and pretty damn uncooperative to begin with. Medical history: Yes, and this is where it gets complicated because Marvel Comics character. Basically, Tom got shot in the face and shoved down an elevator shaft, and underwent an experimental medical procedure where wood-like material was grafted onto his body. Somehow, this was invasive and started to crowd his actual healthy cells and started killing him after a few months of coping well with it, sort of like cancer. From THERE he cut off his hand and replaced it with Deadpool's to try and use the healing factor in Deadpool's blood to slow the change, after much medical "experimentation" by a Dr. Killebrew. This didn't work. He turned into a tree. Then he died. But maybe not. Because comics.

Next time he showed up, he was 100% human again, no grafted material, and his only explanation was "it's a long story". What I headcanon happened is that the tree version of him that died was a doppelganger (which he could canonically create) and that he underwent some more magic medical procedures that killed the plant virus thing and slowly regenerated his own healthy cells via some sort of stem cell healing and/or magic.

Somehow he turned into a tree again at some point, only this time it was a "secondary mutation" because Chuck Austen can't research the canon he's inheriting, but when Wanda removed mutant powers he reverted to 100% human. He also claims he was 100% crazy when he was a tree and had no control over his actions.

Tom also has a limp because his femur was badly broken and ligaments in his leg torn when he was in his twenties. This has not been retconned away.

History of Disease: See above. He's prone to overheating but other than that he's no more or less resistant to disease than the average man his age.

Alcohol and frequency thereof: Ruffnut does drink, usually a couple drinks at night but nothing too crazy unless it's a party. Then she might go a bit overboard and pass out somewhere.
Deaths: 1st Death: Silent Hill arena: Arm ripped off, then eaten alive by Kevin
Condition: Ruffnut is a skinny, lanky but sturdy young girl. They make em tough in Berk and years of cold brutal weather and living off a high protein diet has left her strong and durable despite being clumsy and covered in scars. She has clearly been banged up a brutalized a few times.

Also being separated from her twin has caused her some stress she won't admit to, but studying the tapes of her three arenas should give enough hints to make that deduction.

History of Disease: As mentioned, he has a mutagen virus which is blood- and waterborne, somewhat similar to hepatitis A (non-body fluid transmission part) and B (blood-borne part). It slowly compromises the immune system in Donatello's case, counteracting with his pre-existing mutation; it will transform him into a monster before breaking his DNA down completely and killing him. Things like radiation or serious illness will hasten the degradation. The only thing currently keeping the mutagen in check is the fact he keeps dying in the Arena.

And I guess technically the original mutagen in his blood is a disease of a sort? A type of radiation poisoning in and of itself, in a sense, only it mutated him to be intelligent and human-like.

Deaths: She was horribly injured when sent back to the arena after her return to the capitol. She lost an eye which had to be regrown and threw herself onto a bomb to save her tribute and eventually girlfriend. The Capitol pulled her out before she died and stitched her back together but her brain suffered some minor damage from it as well. Recently she caught the plague that was going around and had horrible fever and chills for a few weeks.
Condition: Fit and healthy, she trains herself every day to keep firm and tight and swift.
Medication: She is currently self medicating on Morphling and some medication to balance out her emotions. Between that and the wine when she is feeling stressed she has alot in her system. Lately though she's been easing off on the medicines.

After that she spent the rest of the year trying to be the best mentor she could be. There have been some missteps but for the most part she's been less stressed as time goes by.

Immunizations: Up to date prior to his arrival, has not received any specific to Panem.
Medical history: None.
History of Disease: His immune system is very good due to being very aware of his dietary needs. No known allergies.
Emergency Contact: He'll put down Stephen Reagan, D6's escort as his contact, but it is a strictly business relationship.
Have they recently experienced any life changes or unusual stress?: He will jokingly say "Dying," and then laugh a little, in a very I see what you did there sort of way. And the serious answer would be a no.
Special Needs: He sometimes has bouts of insomnia because of trouble shutting his brain off, so at most he'd request a harmless sleep aide (melatonin or whatever the equivalent would be, assuming you can't get it over the counter in the Capitol).
Disabling Conditions: It doesn't impede his ability to function, but he has combined-type ADHD, and while not diagnosed with it at this point in his life, he also has Borderline Personality Disorder. I'd be game for her catching this, if relevant.
Notable Scars: No large ones, but a lot of fine cat scratch sized ones all over that are very old. A couple scars shaped like lighter and cigarette burns. All products of child abuse but it could also be speculated some are self-harm even though they aren't.
Other Notable Attributes: His name abbreviated is "enigma" and no one as brought that up yet. Someone has to notice, right?
Demeanor: It's business, so he will be forthcoming with information and perhaps try to joke around a bit. The flirting is disregarded unless it's any sort of compliment to his intellect, and from his emergency contact alone, it should be apparent that forming relationships with others is not his strong suit. If he has nothing to busy himself with (pen and paper, some sort of puzzle, etc.), he will more than likely start to fidget and become anxious unless she provides something with which to challenge him mentally. However you think she'd work him to her advantage.
